Pavement has been damaged

Pavement is uneven and damaged. 4x4 driving over this has caused previously damaged pavement to crack. It’s no unsafe for pedestrians as well. Issue has been present for Approx 2 months but looks to have been fixed before I moved in as well. Damage is on entrance to driveway
